Chapter 2. Patient Assessment Show
A comprehensive head-to-toe assessment is done on patient admission, at the beginning of each shift, and when it is determined to be necessary by the patient’s hemodynamic status and the context. The head-to-toe assessment includes all the body systems, and the findings will inform the health care professional on the patient’s overall condition. Any unusual findings should be followed up with a focused assessment specific to the affected body system. A physical examination involves collecting objective data using the techniques of inspection, palpation, percussion, and auscultation as appropriate (Wilson & Giddens, 2013). Checklist 17 outlines the steps to take. Checklist 17: Head-to-Toe Assessment
How do you measure muscle strength in nursing?A common method of evaluating muscle strength is the Medical Research Council Manual Muscle Testing Scale. This method involves testing key muscles from the upper and lower extremities against gravity and the examiner's resistance and grading the patient's strength on a 0 to 5 scale.
When assessing muscle strength which element does the nurse note as part of a thorough musculoskeletal examination?Terms in this set (37) When inspecting a patient's musculoskeletal system, which muscle characteristic would the nurse assess? The nurse would assess the patient's muscle symmetry as part of a musculoskeletal inspection.
What 3 techniques are used during a musculoskeletal assessment? To perform an examination of the muscles, bones, and joints, use the classic techniques of inspection, palpation, and manipulation.
When assessing muscle tone and strength the nurse would document expected findings as?When assessing muscle tone and strength, the nurse would document expected findings a: "upper and lower extremity muscle strength is 5/5 bilaterally", 5/5 (100%) normal muscle strength with complete ROM against gravity and full resistance.
